You actually need a series ; either it be a game name or whatever. Also, reading your app, there's literally no relation to slenderman's apperance, so why are you trying to claim him?

Just a hint,

You can claim Slenderman if you wish. He comes from mythology and ghost stories so technically he has no series, so you are correct in saying 'no series'. However, because he is from the game 'Slender' you are better off putting that for the series because it is easier to look up.

And yes, you need to actually use his appearance in your application via an image. Just writing his likeliness is not enough as we read your application for proof of its use.

Remember you can reserve face claims. Read the Face Claim list rules to gain a better understanding of this.
