Your Character: (Name of the character who is to kill the applicant)
Your Character Link: (A link to your character)
Death location: (A link to the thread in which the character is killed)
Extra Information: (Other information that may be important as to why the character is killed. If it without the owner's permission please give a strong argument on why the character should be killed without permission of the owner. In which case only the head admin can approve this.)
